Lancet Extractors usage instructions:

Use a lancet needle to poke a tiny hole into one area at a time. The lancet should slide easily into the top. These are all dead cells in the elevated part of the area, so the client or you should barely feel the lancet at all. To do this, hold the lancet between your thumb and forefinger and poke the centre of the whitehead, pus area, pimple or milia gently. One small hole is all you need. Use a mirror if this is on your face or hard to see.

Place large shallow cup extractor on the area and press down firmly. People have a tendency to be too aggressive in trying to remove debris and end up causing a lot of inflammation, which makes things worse. The key is to not put too much pressure on the skin by dragging the tool across the surface of the skin, which can cause tearing of the skin.

Slowly push the extractor tool over, still pressing down firmly against the skin. Repeat this several times, if necessary, to get out all the pus or white keratin built up in the skin. Do this for each effective area.
